This wizard produces a draft for discussion with your huisarts. It is not a finalised wilsverklaring. Legal weight comes from your doctor's discussion and signature. Content reflects Dutch law as of May 2026 and is being reviewed with NVVE and KNMG sources.
Why this exists
When you cannot speak for yourself, doctors look for written wishes. A wilsverklaring written down — and discussed with your huisarts — has legal weight in the Netherlands.
What this wizard does
Produces a structured draft you can read, edit, print, and bring to your huisarts. The discussion with the doctor is what makes it work.
What this wizard does not do
- It does not replace your huisarts.
- It does not replace a notaris for related documents (testament, levenstestament).
- It does not guarantee that every hospital will follow it in every situation.
